Thank you for your willingness to support the work of the Foundation by making monthly contributions. By signing the donation form, you hereby request and authorize the Foundation to initiate monthly charges to your credit card account as indicated on the donation form. Charges will be made on or about the 1st day of each month.

This agreement will remain in full force and effect until the Foundation receives written notice of your intent to terminate 30 days prior to the next charge date. Termination notices should be sent to the Foundation at the address in the letterhead, faxed to 310-396-2127, or e-mailed to staff@liliclairefoundation.org.

You agree to allow the Foundation to recover processing costs associated with charges refused by either party’s credit processor.

You may have other rights regarding this agreement as specified by Federal or State law.

The information in this agreement is confidential and will not, without your prior written authorization, be shared with anyone other than Foundation financial staff and your credit card processor.

Tax deduction receipts will be available in January of each year for the prior calendar year and will be mailed to you. The Foundation is a 501(c)(3) charitable organization, and contributions are deductible to the extent provided by law. Please check with your tax advisor for specific information.
